a 3 inch hole can be cut in a 11 7/8 engineered joist as long as the Centre of the hole does not exceed 1 foot 3 inches from the edge of the bearing structure. This looks like they went past that.

1) You would fail inspection. You cannot have a Sanitary Tee on it's side. Ever.

2) Run the water to test for leaks? Are you serious? Um, you have to have an inspection on that rough in! This means the system has to be air tested at 5lbs, for at least 15 minutes.

Running the water does not count. replace that San T with a Wye. A San T used for drain and waste must be positioned to receive horizontal flow and direct vertical.
